How do I go about getting a supervision order lifted?

When my daughter was 9 months old events led to social services stepping in and removing her from my care. She went to my mum and my mum got a special guardianship and residency order, and all contact between me and my daughter was tk be supervised by either of my parents. I had really bad depression after losing custody and developed a heroin habit I'm ashamed to say. It took me 12 years to get clean. My daughter is now 15 and I see her all the time. However I'd love to take her out for the day and have her stay over at mine etc. My daughter has asked me several times and my mum is happy now that I'm capable of taking care of her. However I have no idea where to start, social services aren't active in our lives and I don't know who to approach about getting the supervision order lifted or even how to go about it. Any advice would be greatly appreciated!
