Observation templates for coordinators

Hi all,
Does anyone have a template that you use for observations of coordinators, including opportunities for reflection on practice, that they are able to share?

Here's the one we use for FGC observation in Coventry, we also have templates for the meeting with referrer and with parent/person with PR. I can share those if they would be useful.
Our documents are sent to the Quality Assurance team which is why there is a judgement on there. We deliberated about how best to apply this when we first started.
